Position Details:
• Job Title: Quality Care Coordinator

• Work Schedule: Monday – Friday

Qualifications:
• Experience in healthcare administration, clinical care (RN,LPN,CMA), or quality assurance preferred.
• Proficiency in Microsoft Office Suite (Word, Excel, PowerPoint) and data management systems preferred.
• Demonstrated ability to manage multiple projects, prioritize tasks effectively, and meet strict deadlines.
• Highly self-motivated with the ability to work independently.
• Strong written and verbal communication skills, with the ability to clearly present information to diverse audiences.
• Proven ability to work effectively with diverse patient populations and interdisciplinary healthcare teams.
Responsibilities:
• Partner with Care Teams to identify and close care gaps in alignment with value-based care initiatives.
• Distribute and analyze performance reports to monitor provider and organizational quality metrics.
• Manage and track quality measures to ensure compliance with payer and ACO requirements.
• Conduct detailed chart audits to validate documentation and improve measure accuracy.
• Collaborate with insurance companies and ACO partners to meet quality improvement and performance benchmarks.
