Position Details:
Qualifications:
· Experience in healthcare administration, clinical care, or quality assurance preferred.
· Proficiency in Microsoft Office Suite (Word, Excel, PowerPoint) and data management systems preferred.
· Demonstrated ability to manage multiple projects, prioritize tasks effectively, and meet strict deadlines.
· Highly self-motivated with the ability to work independently.
· Strong written and verbal communication skills, with the ability to clearly present information to diverse audiences.
· Proven ability to work effectively with diverse patient populations and interdisciplinary healthcare teams.
Responsibilities:
· Partner with Care Teams to identify and close care gaps in alignment with value-based care initiatives.
· Distribute and analyze performance reports to monitor provider and organizational quality metrics.
· Manage and track quality measures to ensure compliance with payer and ACO requirements.
· Conduct detailed chart audits to validate documentation and improve measure accuracy.
· Collaborate with insurance companies and ACO partners to meet quality improvement and performance benchmarks.
