CHANGE, Inc Family is hiring a Full Time Patient Care Coordinator

Position Details:
• Job Title: Patient Care Coordinator
• Location: Weirton
• Work Schedule: Monday – Friday

Qualifications:
• Active LPN Licensure in WV and OH
• Excellent organization, critical thinking, communication, interpersonal and multi-tasking skills, excellent documentation, and computer skills.
• Applicant must be able to work well with diverse patients and staff and promote a positive image for CHANGE, Inc.
• Must maintain a valid driver’s license.
• Pass a background check and drug screening.

Responsibilities:
• Coordinate patient care with a Family Medical Care Provider and assist the medical team with any other duties deemed necessary.
