What to Bring to Your Appointment

You will need to provide the following information:

  • Up-to-date Insurance Card and/or Medicare/Medicaid information if applicable.
  • Proof of Income (if qualifying for the Discount Program).
  • Personal information for you and your family such as Social Security numbers, addresses, and emergency contacts.
  • Co-pay for insurance (due at check-in).
  • Payment for your appointment and medical services (due at the end of your appointment). Payment plans are available through the Billing Office, but a portion must be paid at the visit if utilizing the Discount Scale.
Canceling Your Appointment

We understand that at times it may be necessary to cancel an appointment, or to reschedule. We ask that you call the offices ( Weirton, Wintersville, or Newell call 304-797-7733 ) at least 24 hours prior to your scheduled appointment to cancel or reschedule.
